Detox with Subutex (buprenorphine) - Britlofex (Lofexidine)
This is a one to three week programme for patients who are strongly committed to be detoxified and have enough family support in an appropriate environment.

7 Day Inpatient Detox plus Naltrexone Implant in Alexandria
These are rapid and semi-rapid opiate detoxification inpatient programmes under sedation (not under anesthesia), in a state of the art hospital. As some patients could need a longer period to be detox or to recover, these programmes are flexible and can be tailored to the patient's needs.

6 Month Naltrexone Implant
The purpose of the implant is t provide a blockade against opiates for approximately 6 months. Naltrexone implants are not licensed by the MHRA at present due to there not being enough medical data. However, they are available on a Special Access Scheme.

Subutex (buprenorphine) Detoxification


You can undergo an opiate detoxification with Subutex. This procedure usually takes one to three weeks depending on your clinical condition, your primary drug problem, the amounts you are taking, ploly drug use and last but not at least your liver function.

Pros
- This detox is cheaper than an inpatient rapid opiate detox under sedation.
- Safer than other detox procedures, since Subutex can block any other opiate you can use on top.

Cons
- Subutex is a controlled drug, therefore daily dose must be taken sublingual under supervision, and it takes up to three minutes to dissolve under the tongue.
- Pharmacists have to be licensed to supervise consumption.
- You can feel some discomfort during the first and/or second day.
- Detox last doses and 48hs interval until the first Naltrexone dose are the critical points.
- As any other detox, it is very difficult to stay clean until you have your first naltrexone dose.
- Most of the people relapse during the last week, when Subutex is reduced below 4mg per day.
- As any other detox procedure there is some post detox overdose risk involved.

Britlofex (lofexidine) Detoxification

You can undergo an opiate detoxification with Britlofex and some other symptomatic relief medications. This procedure usually takes two to three weeks depending on your clinical condition, your first drug problem, the amounts you are on, drugs combination and last but not at least your liver functions.

Pros
-This detox is cheaper than an inpatient rapid opiate detox under sedation.
-You don’t need to take it under supervision.

Cons
-Blood pressure has to be closely monitored.
-Most of the patients feel some discomfort during the first and/or second week and need to be prescribed symptomatic relief medications (sleeping meds, pain relief, etc).
-Doses are progressive and have to be taken at least three times a day.
-Detox last doses and 48hs interval until the first Naltrexone dose are the critical points.
-As any other detox of this kind, it is very difficult to stay clean until you have your first naltrexone dose.
-Most of the people relapse during the last week, when Britlofex doses are reduced.
-As any other detox procedure there is some post detox overdose risk involved.

Rapid Opiate Detox Under Sedation plus Naltrexone Implant

This procedure has to be carried out in a clinical inpatient environment for the safety of the patient. It last 5 to 7 days. It is faster and safer than a home detox if carried out properly by professionals with expertise in this procedure.

Pros
-Is quicker and safer than any home detox procedure.
-Relapse rate one month after is many times superior to any out patient detox because naltrexone implant blocks any taken opiate.
-Naltrexone implant procedure is carried out whilst the patient is at the hospital.
-For that reason there is no relapse risk during the 72 hours interval since the last dose of Subutex and the first Naltrexone dose.

Cons
-Is more expensive than home or outpatient detox.
-You need to be hospitalized.
-You have to be fit (not suffering from any major heart, liver or other clinical condition) to undergo the rapid detox and implant.
-Implies minor surgery.

Why Rapid Opiate Detoxification (ROD) and a 6 Month Naltrexone Implant (6MNI)?

4- The patient has made the decision to quit drugs, but it is very difficult to achieve in his "environment", where other users and dealers make drugs easily accessible.

5- To stay clean and fit for a Naltrexone Implant.

6- The patient is at high risk of overdose, and opiates need to be blocked, but compliance with oral Naltrexone is very poor.
